As the COVID vaccine rollout continues, there are ongoing debates about whether you should laminate your vaccine record card or if vaccine passports should become a requirement for access to certain places.

In any case, your COVID vaccination card is likely to be an important document in the months ahead. Travel executives have said they believe people will need to show their card in the future to fly, and it’s possible that some employers will also ask for proof. Some colleges are already requiring vaccinations before a return to campus.

The Centers for Disease Control and Prevention (CDC) recommends you take a photo of the front and back of your card, while Gov. Phil Murphy has advocated laminating your card — something experts are divided on because it would stop you from adding dates of any future COVID booster shots.

But what happens if you lose your card?

The CDC says you should contact your vaccine administrator.

CVS, for example, said it can reissue cards.

“The pharmacy where the patient received their vaccination can reissue a card after validating their identity with proper ID,” spokeswoman Tara Burke said. “The pharmacy will confirm their record on file and reissue.”

Or you can also contact the state for a copy of your records.

“If an individual lost their COVID-19 vaccination card, they will need to request a copy of their immunization record by visiting the New Jersey Immunization Information System (NJIIS),” said Health Department spokeswoman Dawn Thomas. “All COVID-19 doses should be entered into NJIIS within 24 hours, but it may take up until 72 hours depending upon how the data is submitted into NJIIS.”

At the link, click “Submit a Request” on the top blue ribbon.

At the bottom of the page, select “I want to request a copy of my immunization record from NJIIS.” This will open up a form for you to complete.

You only need to complete the required fields, such as your name, address, email and phone.

In the required field that asks for your “Facility/Organization Name,” you can enter “n/a,” Thomas said.

You will need to attach a copy of your photo ID.

“Acceptable forms of ID include: a state-issued photo driver’s license with address; a state-issued photo non-driver’s identification card with address; a similar form of identification issued by this State, another state, or the Federal government; or a photo identification card issued by a New Jersey County Clerk,” the website says.

If you specifically want a copy of the lot number for your COVID-19 vaccine, add that request to the description section, the site says.

The state didn’t say whether some vaccination sites, such as the mega-sites, can reissue cards at their physical locations.

The Department of Health says most requests are typically answered within 24 to 48 hours, and your records will be mailed to you. The records cannot be emailed, it said.
