If you lost your COVID-19 vaccination card, have no fear — your records aren’t lost in a great abyss somewhere.

“It’s certainly like any part of your medical record,” explained Bob Hawkes, Director of the Physican Assistant Program at Florida Gulf Coast University. “It’s verification that you received a certain vaccine similar to when you receive other immunizations.”

Overall, the CDC does not keep vaccination record information. Instead, those who have lost their cards can either contact the facility that administered the shot or contact the local or state immunization information program through the state’s health department.

Lee County: If you got your vaccine at a Lee County vaccination site but lost your card, you can send an email to [email protected] In the email, include your date and location of your first dose, name, date of birth, address, and phone number.

NBC2 has reached out to Collier and Charlotte counties and is waiting for a reply.

Participating Pharmacies

Not all pharmacies handle vaccination information the same way. Some can reissue a vaccination card upon ID verification, while others can only give a printed vaccine history summary. Either way, it’s proof of vaccination in some form.

Publix: Publix cannot provide an additional vaccine card once the first one is lost, according to a spokesperson. However, a customer can visit any Publix Pharmacy and request their customer history summary.

CVS Pharmacy: CVS can reissue a vaccination card after validating identity with proper ID, a spokesperson said. The pharmacy will confirm the record on file and then reissue the card.

Walgreens: Walgreens maintains digital copies of patient vaccination records and can verify vaccination status on behalf of patients, a spokesperson confirmed. A verifiable digital dose card will be available in May to anyone who received their COVID-19 vaccine from Walgreens. It will be accessible through the myWalgreens website and app.

NBC2 has reached out to several other participating pharmacies and is waiting to hear back.

Loss Prevention

As far as keeping your card safe — if you haven’t lost it already — the CDC listed some tips and tricks on their website.

As soon as you get your vaccine card, take a picture of it, the CDC recommended. You should also make a photocopy if that’s available to you. However, be mindful not to post a photo of your card to social media, as it contains sensitive personal information.

You could also laminate your card, but be careful not to smudge the ink. If you go this route, wait until you’re completely vaccinated because you’ll need to add additional shots to the record.
